EMORY SPORTS MEDICINE CENTER - Outreach MD Referral Policy and Procedure:
To schedule to see one of our Emory Sports Medicine Physicians follow the steps below:
1. Secure Online form for Registration: (preferred method)
Please scan the following QR code or follow the link below to submit the information needed to schedule an appointment. Once the form is sent the scheduler will call to schedule. This is the best way to schedule as it is secure and time efficient. This can be done via smart phone, tablet, or computer.

2. Outreach phone (404-783-9566)
Calls taken from 8am–8pm Mon-Thurs, 8am–2pm Fridays during football season and 8am–5pm Mon-Fri otherwise, to coordinate expedited appointments from our outreach community. Messages can be left 24 hours a day and weekend calls will be returned Sunday afternoon/evening based on urgency or first thing Monday morning.
Our community includes high school, college, semi-professional teams and recreational teams. Please provide this phone number to parents, coaches, athletes, etc. when an injury occurs and you feel they should be seen in our clinic in 24–72 hours. Please note phone calls without messages will not be returned. Please route all new patient calls to this phone and not through the physician’s office.

In the event an athlete cannot use the above form, you can take a picture of the completed original form, insurance cards, etc. and email them. We figured that most of you have smart phones and can do this from the field and/or training room. After sending an email, please text the outreach phone stating that the information has been sent (outreach phone does not receive emails). After receiving the text, I will login to a computer to retrieve the information. If you do not have a form handy, simply write the info on a piece of paper and send that or call the outreach number with the information.
At a minimum we need the patient name, date of birth, parent name and number, injury information and school/group they participate with.

3. MD wants to expedite process:
If you see someone and want to see them the next day (or Monday if seeing over the weekend), please work with the athletic trainer to have them send the link to the patient. You can also text the outreach phone the name of the patient and injury and how soon you would like to see them. We are trying to make sure it is what you want and not just a parent saying I need an appointment today, etc. We are trying to utilize your schedule openings versus any overbooking.
